Different people react to depression in different ways. Some people lose their appetite, while others eat more. The reason for this is that different parts of the brain control appetite variation in depressed patients. Depression-related decreases in eating are associated with hypoactivation of min-insula areas of the brain that controls the body’s physiological state. Depression-related increases in eating are linked to hyperactivation of the reward circuitry of the mesocorticolimbic system, which controls our sense of pleasure and enjoyment.

While it may seem difficult to change your own brain chemistry, being aware of the problem is the first step. When you detect a change in your appetite, taking the time to assess and chart a course of action is critical. One of the best ways to manage your appetite while depressed is to meal plan. This is helpful for people who suffer from both an increase and a decrease in appetite. For those who find themselves overeating, having set meals in individual containers can help with portion control and self-regulation of how much you’re eating, rather than sitting around with an open bag of chips.

Weight gain and antidepressants: Emotional eating, indifference and effects on appetite.

Antidepressants such as SSRIs can cause weight gain in some people and weight loss in others. We explain how that works:
1. Weight loss is often about their effect of SSRIs on increasing indifference which may then reduce emotional eating.
2. Weight gain is with SSRIs is often about their effects on increasing someone’s indifference in their food choices.

Other antidepressants like Tricyclic antidepressants and Mirtazapine can cause sedation because of their antihistamine effects on increasing appetite. Thats why some medications which cause drowsiness also cause increased appetite.

SSRIs include aropax/paroxetine, zoloft/sertraline, prozac/fluoxetine, luvox/fluvoxamine, lexapro/escitalopram and cipramil/citalopram. The mostly interact with the SERT protein to increase serotonin residence in the synapse. They minimally affect the Histamine receptor.

Tricyclic antidepressants (TCA) include endep/amitriptyline, anafranil/clomipromine, dothiepin, nortriptyline etc. Generally have significant effect on Serotonin and noradrenaline reuptake (SERT and NET) as well as hitting specific serotonin receptors (e.g. clomipramine avidly binds 5-HT2c and 5-HT6). They bind Alpha1 and potently bind H1.

Mirtazepine (Avanza) is a tetracyclic antidepressant, also called a “noradrenergic and specific serotonergic antidepressant”, but it’s not very specific. While it has negligible effect on serotonin or noradrenaline reuptake (negligible SERT or NET effect) but strongly interacts with a number of other receptors including serotonergic 5-HT2C, 5-HT3, Alpha2A, Alpha2C and H1.

Key facts
– Not everyone gets diabetes symptoms, so taking metformin may not make you feel any different. That does not mean it is not working, so keep on taking it.

– It’s best to take metformin with a meal to reduce the side effects.
The most common side effects are feeling and being sick, diarrhoea, stomach ache and loss of appetite. Vitamin B12 deficiency is also a common side effect of taking metformin in higher doses or for long periods.

– Metformin does not cause weight gain, unlike some other diabetes medicines.

– If you have diabetes, you can get free prescriptions for all of your medicines, not just your diabetes ones. You’ll need to have a medical exemption certificate.

I'm pregnant, and losing my appetite and weight. What can I do? Will this hurt my baby?

You mentioned that you’re 16 weeks along, and up to this point, you’ve lost 6 pounds because you’re just not interested in eating, it’s more of a lack of appetite. And this could be due to the nausea that you’re feeling during pregnancy. And if that’s the case, talk with your OB provider, and number 1, they’ll be able to tell you if this weight loss is okay for you, based on your pre-pregnancy weight, and number 2, they may be able to prescribe a medication that will help you get over the nausea so that you’ll want to eat. But as for what to eat to increase your desire to eat, that’s entirely up to you. If something sounds good, then eat it. But you should fill your diet full of whole grains, lots of fruits, lots of vegetables, lean protein, low-fat dairy, and healthy fat. So if you can, choose a protein and a good healthy carbohydrate, like fruits or a wholegrain, to eat with each meal and snack.

And maybe try eating more frequent and smaller meals throughout the day instead of eating 3 large ones. And you might find that just keeping your stomach a little bit full all the time with a protein and a healthy carbohydrate will also ease your nausea, and maybe help you eat the amount of calories you need, and gain the appropriate amount of weight. The amount that you should gain during pregnancy is based on your pre-pregnancy weight, and again, your doctor will be able to advise you on what that is for you, based on their knowledge of your situation.

Don’t forget to keep taking a prenatal vitamin too. A lot of people think you just take that while you’re trying to conceive, but it’s also important to take over the course of the entire pregnancy and while you’re breastfeeding, because it helps to ensure that you and your growing baby have the nutrition you need. So that’s important too, in addition to a healthy diet.
